Please complete this form to initiate your membership cancellation. One form submission is required per member—if you are cancelling multiple memberships, you must complete the form separately for each member. Cancellation requests must be submitted at least seven (7) days prior to your next renewal date. Memberships cannot be cancelled within the first 14 days of sign-up. You will receive an email and text confirmation once your cancellation has been successfully processed.
